Please be Aware: St. Edward the Confessor School follows Syosset School District protocols regarding COVID-19 if your child is symptomatic while at school, they will not be able to return until they have been evaluated by a medical professional.
Please find below all of the necessary forms needed in order for your child/children to attend school. These are used and approved by the Syosset Central Schools along with important information to maintain a healthy and safe environment for all our students.

As required by New York State Education Law, all New Enterers, Nursery, Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, and 9th-grade students must have physical examinations done within 12 months of the first day of school.
In accordance with the New York State Immunization Laws, a Certificate of Immunization listing specific dates, signed and stamped by a physician, must be on file for each student. The student must be immunized against Diphtheria (3 doses), Polio (3 doses), MMR (1 dose for Nursery and Pre-k, 2 doses (K-8), Hepatitis B (3 doses), and Varicella (2 doses). Students will be excluded from attending school in September if they have incomplete or missing immunization records (as per NYS immunization Law).
If your child is sent home or stays home due to an illness or becomes ill in the evening, it is expected that he/she remains symptom-free (fever less than 100, nausea, vomiting) for 24 hours before returning to school. WITHOUT THE AID OF MEDICATION SUCH AS TYLENOL/ADVIL, KAOPECTATE, ETC.
Annual screenings are done for vision ( Grades K,1,2,3,5,7) and all new students (K-8). Hearing (Grades K, 1, 3, 5, 7) and all new students K-8. Scoliosis will be done for grades 5-8 if a physician hasn’t already done so.
